Orforglipron is a new GLP-1 weight loss pill developed by Eli Lilly. Known as Foundayo in the US, where it’s been approved by the Food and Drug Administration (FDA), it could become an important oral alternative to injectable treatments like Wegovy and Mounjaro.

Although Orforglipron is not currently approved or available in the UK, its development is significant. Clinical trials suggest this daily tablet may support meaningful weight loss, while also being studied for weight maintenance and wider health measures such as blood sugar, blood pressure and cholesterol.

In this guide, we’ll explain how orforglipron works, when it could become available in the UK and how it compares with other weight loss treatments.

What is Orforglipron?

Orforglipron, known as Foundayo in the US, is a GLP-1 receptor agonist developed by Eli Lilly, the company behind Mounjaro.

GLP-1 is a natural hormone that helps control appetite, fullness and blood sugar after eating.

Orforglipron and tirzepatide, the active ingredient in Mounjaro, are not the same medicine. Tirzepatide is a weekly injection that works on both gastric inhibitory polypeptide (GIP) and GLP-1 receptors, while orforglipron is a daily tablet that works on GLP-1 receptors.

Orforglipron is also a small-molecule, non-peptide medicine.

This means your body can absorb it properly when you take it as a tablet, which may give you more choice when comparing future weight loss treatment options.

How does Orforglipron work?

To understand orforglipron, it helps to begin by understanding how GLP-1’s work.

GLP-1 is a natural hormone your gut releases after you eat. It sends signals between your brain, stomach and pancreas to help control appetite, fullness and blood sugar. Orforglipron works by copying this GLP-1 signal.

This may support weight loss in four main ways:

  1. It may reduce hunger and cravings, including “food noise”.
  2. It slows how quickly food leaves your stomach, helping you feel fuller for longer.
  3. It may help you feel satisfied with smaller portions.
  4. It supports blood sugar control by helping your body release insulin when blood sugar is high.

Together, these effects may make it easier to eat less, manage cravings and maintain a calorie deficit alongside healthy diet and regular exercise.

Is Orforglipron available in the UK?

Orforglipron isn’t currently available in the UK. It’s been approved in the US as Foundayo by the FDA but still needs approval from the UK Medicines and Healthcare products Regulatory Agency (MHRA) before it can be prescribed here.

Some reports suggest it may become available privately in late 2026 or early 2027, but this is currently an estimate and can’t be guaranteed.

UK pricing hasn’t been confirmed. In the US, Foundayo pricing starts at $149 per month for the lowest dose, with many patients expected to pay around $349 per month.

Even if Orforglipron is approved in the UK, it may not be available straight away. Approval, launch, supply, pricing and prescribing are all separate steps.

If you’re comparing NHS vs private prescriptions, NHS access would likely take longer. The National Institute for Health and Care Excellence (NICE) would need to review whether Orforglipron works well and offers good value for the NHS before recommending it for NHS prescribing.

How effective is Orforglipron for weight loss?

Although Orforglipron weight loss pills are not available in the UK yet, current evidence is promising.

ATTAIN-1 is the largest Orforglipron study so far. It was a 72-week phase 3 clinical trial across 10 countries and included more than 3,000 adults with obesity, or adults who were overweight with at least one weight-related condition, not including diabetes.

Here’s what the study found:

  • After 72 weeks, people taking the highest dose, 36 mg, lost an average of 12.4% of their body weight, or around 27.3 pounds, compared with placebo.
  • Nearly 60% of people lost at least 10% of their starting weight, and around 40% lost at least 15%.
  • Heart health risk factors also improved, including blood pressure and LDL cholesterol.

Earlier trials also showed positive results:

  • Weight loss after 26 weeks: In adults with obesity, or adults who were overweight with at least one weight-related condition, Orforglipron led to average weight loss of 8.6% to 12.6%, compared with 2% for placebo.
  • Type 2 diabetes: A separate phase 3 trial, ATTAIN-2, studied Orforglipron in adults with obesity or overweight and type 2 diabetes. It found Orforglipron reduced blood sugar levels by an average of 2.1% after 26 weeks, compared with 0.43% for placebo. People in this trial also lost an average of 10.1 kg, compared with 2.2 kg for placebo.

 

These results are promising, but they are still averages. Individual results can vary depending on dose, consistency, lifestyle habits, medical history and personal response.

Orforglipron side effects

Orforglipron can cause GLP-1 side effects similar to other weight loss medicines, especially digestive symptoms. This can happen because GLP-1 medicines slow how quickly food leaves the stomach and affect appetite and digestion.

In an earlier phase 2 weight loss trial, 86% to 90% of people taking orforglipron reported side effects, compared with 76% of people taking placebo.

The most common side effects were:

  • Feeling sick, also called nausea
  • Indigestion
  • Constipation
  • Vomiting
  • Diarrhoea
  • Burping
  • Stomach discomfort

 

Side effects were more likely when people started on a 3 mg dose instead of a 2 mg dose, or when the dose increased quickly every 1 or 2 weeks rather than more slowly every 3 weeks.

This is why GLP-1 medicines often start at a lower dose and increase gradually, to help the body adjust.

In the trial, 35 people stopped taking Orforglipron because of side effects. Overall, Orforglipron had a similar safety profile to other GLP-1 medicines.

Why is Orforglipron different from weight loss injections?

Orforglipron is different from injectable GLP-1 treatments in three main ways:

  1. It’s taken as a tablet: Wegovy and Mounjaro are weekly injections, while Orforglipron is taken once a day as a tablet. This avoids the need for needles, injection pens or sharps disposal.
  1. It has a different structure: Many GLP-1 treatments are peptide-based, but Orforglipron is a small-molecule, non-peptide medicine. This helps it survive digestion and works as a tablet.
  2. It targets GLP-1 only: Orforglipron targets GLP-1 receptors, while Mounjaro works on both GIP and GLP-1 receptors.

However, oral treatment does not automatically mean better, safer or more effective. Suitability would still depend on medical history, side effects and prescriber guidance.

What are the benefits of an oral GLP-1 pill?

An oral GLP-1 pill may be easier to fit into daily life than an injection. Tablets are more discreet, may simpler to store and easier to carry when travelling.

Current information suggests Orforglipron can be taken at any time of day, with or without food, and without strict water or timing rules. This may make it easier to build into your routine than some other oral GLP-1 medicines.

However, tablets won’t automatically be the best option for everyone.

Some people may prefer a weekly injection, while others may find a daily tablet easier to remember.

Orforglipron for weight maintenance

When you lose weight with GLP-1 treatment, the next question for many is how to keep that weight off. This is why researchers are studying GLP-1 medicines not only for initial weight loss, but also for longer-term weight maintenance therapy.

This may be especially relevant if you’ve lost weight with other weight loss treatments like Wegovy or Zepbound but would prefer a simpler oral option in the future.

The ATTAIN-MAINTAIN trial looked at this question. Adults who had already lost weight on weekly injectable treatments switched to either orforglipron or placebo for 52 weeks.

People who moved from Wegovy to Orforglipron maintained about 79.3% of their previous weight loss, compared with 37.6% in the placebo group. People who moved from Zepbound to orforglipron maintained about 74.7%, compared with 49.2% in the placebo group.

On average, people who switched from Wegovy to orforglipron regained 0.9 kg over the year. Those who switched from Zepbound regained 5.0 kg on average.

These results suggest Orforglipron may have a future role in longer-term weight management, including after switching GLP-1 treatments from an injectable option. However, it isn’t currently approved or available in the UK, and any maintenance use would need to follow UK guidance and prescriber advice.

Orforglipron and linked health conditions

Obesity isn’t just about body weight. It’s a complex, long-term condition that can affect your blood sugar, blood pressure, heart health and sleep.

Excess body fat, especially around the organs, can increase inflammation and affect blood sugar, cholesterol and blood pressure.

GLP-1 medicines may support more than weight loss. Growing evidence suggests they may also help improve type 2 diabetes, sleep apnoea and high blood pressure in adults living with obesity.

This means future treatments may be judged not only by how much weight you lose, but also by how they support your wider long-term health.

Who might Orforglipron be suitable for?

The use of Orforglipron in the UK hasn’t yet been confirmed. However, if approved, it may be suitable for adults with obesity, usually defined as a body mass index (BMI) of 30 or above.

It may also be suitable for some adults who are overweight, usually a BMI of 27 or above, if they also have a weight-related health condition such as type 2 diabetes, high blood pressure or high cholesterol.

If approved, Orforglipron may appeal to people who prefer tablets, dislike injections, want fewer restrictions around food and drink, or travel regularly. However, a prescriber would still need to review your BMI, health conditions, medical history, current medicines and previous treatment response.

Who may not be suitable for GLP-1 medicines?

GLP-1 medicines may not be suitable for everyone.

Depending on the medicine, extra caution may be needed in cases such as:

  • Pregnancy, breastfeeding or planning a pregnancy
  • Personal or family history of medullary thyroid carcinoma, a rare type of thyroid cancer
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
  • Serious allergic reaction to the medicine or any of its ingredients
  • History of pancreatitis
  • Severe digestive problems or gastrointestinal conditions

How to take Orforglipron

Orforglipron is taken as a tablet once a day. In the US, Foundayo can be taken with or without food, and current prescribing information says the tablet should be swallowed whole, not broken, crushed or chewed. It shouldn’t be taken more than once a day.

The dose may start low and increase gradually over time. This is called dose escalation. It helps your body adjust to the medicine and may reduce side effects such as feeling sick, diarrhoea or constipation. The exact dose may depend on why the medicine is being used, how well it works and how well you tolerate it.

Specific UK guidelines aren’t in place yet.

How to store Orforglipron

According to current US product information for Foundayo, Orforglipron should be stored at room temperature, between 20°C and 25°C. It should be kept in its original bottle and carton to protect it from light, and the cap should be replaced after each use.

Storage instructions may differ if Orforglipron is approved in the UK, so this should be checked against official UK product information before publication.
