Should I stick to my Mounjaro dose or move up?

Ideally you should increase your dose after a minimum of four weeks at each dose, but only if it makes sense for your progress.

Many patients achieve good results before reaching the maximum dose and may not need to increase their dose each dose. There is no requirement to increase your dose.

In this guide we’ll explain the full Mounjaro injections dose schedule, when it might be appropriate for you to stick with your current dose and when a higher dose might benefit you.

The full Mounjaro dose schedule

Mounjaro is a once-weekly weight loss injection and treatment always begins at 2.5mg. You must spend a minimum of four weeks at each dose level before moving up.

The table below shows the full schedule, which doses are classed as maintenance doses, this means that you can stay on this dosage for bit longer meaning that you and the weight loss figures reported in clinical trials.

DoseWeeksTypeTypical weight lost
2.5mg1–4 (min. 4)StarterMinimal — body adjusting
5mg5–8+ (min. 4)MaintenanceInitial losses begin
7.5mg9–12+ (min. 4)EscalationSteady progress for many
10mg13–16+ (min. 4)Maintenance~19.5% body weight (average)
12.5mg17–20+ (min. 4)EscalationStrong appetite suppression
15mg21+ (min. 4)Maintenance~20.9% body weight (average)

The 4-week rule

Four week is minimum time at each dose, not the recommended time. Many patients benefit from staying longer on a dose that is working well for them.

Increasing your dosage too quickly increases the chance of side effects without improving weight loss results.

Understanding your current Mounjaro dose

When you begin your weight loss journey with Mounjaro (tirzepatide), our clinicians will start you on a 2.5 mg dose.

This initial dose is not chosen at random; it’s an essential part of the process that is designed to gradually introduce your body to the medication.

Starting at 2.5 mg allows your body to adjust to Mounjaro, helping to minimise potential side effects and ensuring that the medication is well-tolerated.

This approach is critical because Mounjaro works by affecting your appetite and metabolism, which can cause significant changes in your body. By beginning at a lower dose, you give your body time to adapt to these changes in a controlled and safe manner.

Signs that your current dose might be working

If you’re wondering whether your current dose of Mounjaro is effective, there are several positive signs to look out for. Here are some key signs that your current dose might be working:

  1. Reduced appetite

If you notice that you’re feeling full more quickly or that your cravings, especially for unhealthy foods have decreased, it’s a good sign that the medication is helping to regulate your appetite. One of the primary effects of Mounjaro is appetite suppression.

  1. Steady weight loss

A consistent reduction in weight over time, even if it’s gradual is a sign that your body is responding well to the current dose.

  1. Better control overeating habits

You find it easier to stick to a healthy eating plan and resist overeating, especially in situations where you previously struggled. This is a strong sign that your current dose is helping you make better dietary choices.

  1. Increased energy levels

As your body begins to shed excess weight, you might notice an increase in energy. Feeling more energetic and motivated to engage in physical activities.

When to consider increasing your Mounjaro dose

While starting with a lower dose of Mounjaro is important for helping your body adjust, there may come a time when increasing the dose could be beneficial for achieving your weight loss goals.

Here are some situations where increasing your Mounjaro dose might be considered:

  1. Plateau in weight loss

If your weight has been consistently rising or holding flat for three or more weeks despite sticking to your diet and exercise routine, this is a signal that your body may have adapted to the current dose.

If you’re hesitant about increasing your dose then you can stay on your dose for a further month but with a focus on your diet and lifestyle to see if that helps you break through the plateau.

Important: A temporary stall of one to two weeks or fluctuations of 1–2 lbs is not a plateau. Weight naturally varies day to day due to water retention, hormones, sleep, and digestion. Look for a sustained pattern before making assumptions.

  1. Persistent hunger or cravings

If your appetite has returned or you’re struggling with increased cravings, it may suggest that your current dose is no longer suppressing your appetite effectivelyAn increase in dosage might help regain control over your eating habits.

  1. Minimal weight loss despite adherence

If you’ve been consistently following your prescribed Mounjaro regimen, alongside a balanced diet and staying physically active, but seeing minimal weight loss, it could be an indication that a higher dose is needed to improve the medication’s effects.

When to consider reducing your dose

Going down a dose is always an option and sometimes the right one. Not everyone tolerates every dose well, and a lower dose that you can stick to consistently will produce better long-term results than a higher dose you struggle with.

Consider speaking to our clinical team about reducing your dose if you experience:

  • Persistent nausea or vomiting – that does not ease after two to three weeks at the new dose
  • Severe gastrointestinal symptoms – diarrhoea, abdominal pain, or bloating that affects daily life
  • Significant fatigue or dizziness – that doesn’t go away
  • A feeling that the current dose is simply ‘too much’ – excessive appetite suppression that makes eating normally very difficult

Reducing your dose is not a setback. It is a clinical adjustment, and our clinicians can support you through it.

Key considerations before increasing your dose

Before making any changes, it’s crucial to discuss your progress and concerns with our clinicians. They can evaluate whether increasing the dose is the best course of action and help manage any potential side effects associated with a higher dose.

Remember, the goal is to achieve steady, sustainable weight loss while maintaining your overall health and well-being.

Benefits of moving up to a higher dose

Increasing your Mounjaro dosage under clinical guidance can offer several potential benefitsHere are some of the key benefits that might come with moving up to a higher dose:

  1. Enhanced appetite suppression

Higher doses of Mounjaro typically reduce hunger by suppressing your appetite, making it easier to resist cravings and reduce overall calorie intake.

  1. Accelerated weight loss

Increasing your Mounjaro dose could help you break through a weight loss plateau and continue towards your goal.

  1. Greater satiety between meals

Moving up to a higher dose may help you feel fuller for longer periods between meals, reducing the need for snacking and helping you maintain a consistent eating routine.

  1. More significant health improvements

With more effective weight loss and better blood sugar control, you might experience additional health benefits such as reduced blood pressure, improved cholesterol levels, and lower risk of obesity-related conditions like heart disease.

Risks and considerations of adjusting your Mounjaro dose

While increasing your Mounjaro (tirzepatide) dose may offer potential benefits, it’s important to be aware of the associated risks and considerations.

Before making any changes, it’s crucial to understand these factors and discuss them with our pharmacist. Here are some key risks and considerations to keep in mind:

  1. Increased side effects

Explanation: A higher dose of Mounjaro may increase the severity of common side effects including nausea, vomiting, diarrhoea, and abdominal discomfort.

In some cases, these side effects can be more pronounced with a higher dose.

Consideration: If the side effects are severe or persistent, contact us immediately.

  1. Risk of hypoglycaemia

Explanation: While Mounjaro is effective at improving blood sugar control, a higher dose may increase the risk of hypoglycaemia (low blood sugar), especially if you’re also taking other medications for diabetes.

Consideration: Our clinicians may advise you to discuss your other diabetes medications with your specialist/GP as part of a dose review

It’s important to be aware of the signs of low blood sugar, such as dizziness, sweating, confusion, and fatigue.

  1. Possible overdose symptoms

Explanation: Taking too high a dose of Mounjaro could potentially lead to symptoms of overdose, such as severe gastrointestinal distress, extreme nausea, or even more serious conditions like pancreatitis.

  1. Declining results or diminishing returns

Explanation: There’s a point at which increasing the dose may not give you greater benefits, particularly if you have already achieved significant progress at your current dose

The small gain from increasing your dose might not outweigh the additional side effects.

  1. Cost

Explanation: Higher doses of Mounjaro are more expensive and this can add up over months of treatment.

Consideration: If your current dose is delivering results, staying at a lower dose can be sensible financially and clinically.

What to expect after adjusting your dose

Understanding how your body might react can help you manage any new experiences and stay on track with your weight loss journey. Here’s what you can anticipate:

  1. Initial side effects

Explanation: After increasing your dose, you may experience a resurgence of side effects such as nausea, vomiting, diarrhoea, or abdominal discomfort. These side effects are common when starting or increasing a dose of medications like Mounjaro.

What to expect: These symptoms are usually temporary and should subside as your body adjusts to the higher dose. However, if side effects are severe or persistent, contact our team for guidance.

  1. Enhanced appetite suppression

Explanation: A higher dose of Mounjaro is likely to increase its effects on appetite regulation, making you feel fuller faster and reducing cravings.

What to expect: You might notice a more significant reduction in hunger, which can make it easier to stick to a calorie-controlled diet and avoid overeating.

  1. Potential weight loss acceleration

Explanation: With an increased dose you may see a boost in your weight loss progress as the medication exerts stronger effects on appetite and metabolism.

What to expect: Some people may notice quicker or more pronounced weight loss in the weeks following a dose adjustment. Keep track of your progress and celebrate these milestones.

Important: Your weight loss should be gradual so any rapid weight loss will put you at increased risk of complications and you might be advised to lower your dose for your own safety.

  1. Adjusting to the new dose

Explanation: It may take some time for your body to fully adjust to the new dose, both in terms of side effects and the overall impact on your weight loss.

What to expect: Be patient as your body adapts to the higher dose. Give yourself time to adjust to any new feelings or changes and stay in close communication with our pharmacist about your progress.

Final tips

Stay hydrated: Increased side effects like nausea can sometimes lead to dehydration. Ensure you’re drinking at least two litres of water throughout the day.

Monitor your progress: Keep track of your weight, appetite, and any side effects. This information will be valuable for your prescriber in assessing how well the new dose is working for you.

Be patient: Adjusting to a new dose is a process. It might take a few weeks to see the full effects on your weight loss and overall health, so give your body the time it needs to respond.

Tips for maximising results at any Mounjaro dose

Whether you’re just starting on Mounjaro weight loss injections or considering a dose adjustment, there are several strategies you can use to get the most out of your treatment:

  1. Stick to a balanced, calorie-controlled diet

Explanation: Mounjaro helps reduce appetite, but it’s important to pair this with a diet that supports weight loss. Focus on nutrient-dense foods that are low in calories but high in essential nutrients.

Tip: Incorporate plenty of vegetables, lean proteins, whole grains, and healthy fats into your meals. Avoid highly processed foods, sugary snacks, and beverages that can add unnecessary calories.

  1. Prioritise regular physical activity

xplanation: Exercise is a key component of any weight loss plan, helping to burn calories, build muscle, and boost metabolism.

Tip: Aim for at least 150 minutes of moderate-intensity aerobic activity each week, such as walking, cycling, or swimming.

Include strength training exercises two to three times a week to maintain muscle mass and support metabolic health.

  1. Stay consistent with your medication

Explanation: Taking Mounjaro consistently as prescribed by our clinicians, this is crucial for maintaining steady levels of the medication in your body.

Tip: Set reminders to take your dose at the same time each week or follow the schedule provided by our clinicians. Consistency helps maximise the medication’s effectiveness.

  1. Monitor your progress regularly

Explanation: Keeping track of your weight, appetite, and any side effects can help you and your healthcare provider assess how well the medication is working.

Tip: Use a journal or an app to log your daily food intake, physical activity, weight, and any symptoms. Regular monitoring can help identify patterns and areas for improvement.

  1. Manage stress and prioritise sleep

Explanation: Stress and poor sleep can negatively impact weight loss by affecting your hormones and increasing cravings for unhealthy foods.

Tip: Incorporate stress-reducing activities like meditation, deep breathing exercises, or yoga into your routine. Aim for 7-9 hours of quality sleep each night to support overall health and weight management.

  1. Stay hydrated

Explanation: Drinking enough water is essential for overall health and can aid in weight loss by helping control hunger and boosting metabolism.

Tip: Aim to drink at least 8 glasses of water a day, and more if you’re active or live in a hot climate. Sometimes, thirst can be mistaken for hunger, leading to unnecessary snacking.

  1. Seek support from professionals

Explanation: Working with your GP, doctor, dietitian, or nutritionist can provide you with personalised advice and accountability, helping you stay on track.

Tip: Regularly check in with our clinicians to discuss your progress, adjust your treatment plan if needed, and receive support tailored to your needs.

  1. Set realistic, achievable goals

Explanation: Setting attainable weight loss goals can help you stay motivated and focused, making the journey more manageable.

Tip: Break down your weight loss goals into smaller, short-term wins.

Celebrate your achievements along the way, no matter how small. This can boost your confidence and keep you motivated.

Don’t just focus on the scales, you might notice that clothing feels looser, sleep quality improves and you feel more energetic.

Making the right decision for your weight loss journey

Deciding whether to stick with your current dose of Mounjaro or to a higher dose is a significant decision in your weight loss journey.

It’s important to remember that weight loss is a highly individual process, and what works best for you may not be the same as for someone else.

The key to making the right decision lies in understanding your body’s response to the medication, being aware of the potential benefits and risks, and maintaining open communication with our team.

If you’re seeing steady progress and manageable side effects at your current dose, it might be best to continue as is.

However, if you’re experiencing a plateau, persistent hunger, or minimal weight loss, a dose adjustment could provide the boost you need.

Always weigh the potential benefits against the risks, and consider factors like your overall health, lifestyle, and long-term goals.

FAQs

No. Many patients achieve excellent results at 7.5mg, 10mg or 12.5mg and never need to progress further. The maximum dose of 15mg exists for those who need additional support and can tolerate it, but it is not a target. Your goal is effective weight loss with manageable side effects — not reaching the highest dose.

In most cases, yes, though this is assessed on a case-by-case basis. If you have been off Mounjaro for a significant period, you would typically restart at 2.5mg and titrate upward again, rather than resuming at your previous dose.

This gives your body time to re-adjust. Speak to our clinicians about your specific circumstances.

Potential risks include increased side effects such as nausea, vomiting, or gastrointestinal discomfort, as well as a higher risk of hypoglycemia (low blood sugar) if you’re also taking diabetes medications. It’s important to carefully weigh these risks with our pharmacist before adjusting your dose.

Yes, combining Mounjaro with a balanced diet, regular physical activity, and healthy lifestyle habits can significantly enhance its effectiveness. Proper hydration, stress management, and consistency with your medication are all important factors in maximising your results at any dose.

Weight loss plateaus are common and can be frustrating. If you hit a plateau, consider adjusting your diet, increasing your physical activity, or reviewing your medication regimen with your healthcare provider. Sometimes, even small changes can help you break through the plateau and continue losing weight.
