Dutasteride Vs Finasteride: Which is Better?

 

Hair loss can be distressing for many. And for many this may prompt an exploration of treatments; finding what works best for you. Two medications commonly prescribed for the most common type of hair loss (androgenic alopecia (male pattern baldness)) are finasteride and dutasteride. Similar treatments from the same family of medications, but how do they actually work, and which one is better?

Understanding the mechanism

Both finasteride and dutasteride belong to a class of drugs known as 5-alpha reductase inhibitors (5-ARIs) which work by blocking the conversion of testosterone into dihydrotestosterone (DHT). This DHT is the main hormone involved in hair loss. Heightened levels leading to a receding hairline, thinning hair around the crown, and eventually hair loss.

The difference between the two medications is, while finasteride works by stopping the action of one of these hormones (type II), dutasteride actually stops both type I and type II. Leading to a more significant drop in DHT levels.

Key differences:

While dutasteride and finasteride are within the same family of medications, and have many similarities, there are a few differences to be aware of that may affect your preference of one over the other.

1. MHRA approved in the UK:
  • Finasteride is approved by the MHRA (Medicines and Healthcare products Regulatory Agency) in the UK for the treatment of male pattern hair loss. Dutasteride, on the other hand, is not explicitly approved for this purpose. It is sometimes prescribed for this use off-label, but this would mean that information in the patient information leaflet, or any information found in the British National Formulary (BNF) would not apply to this use.
2. Effectiveness:
  • Some studies have suggested that dutasteride may be more effective than finasteride due to the way that they both work, as mentioned previously. However, the exact effectiveness still needs more research to confirm conclusively that one outperforms the other.
3. Availability:
  • Due to the MHRA-approval of finasteride, this medication is readily available on prescription from most online pharmacy retailers. Dutasteride, however, due to not being formally approved for this use, may not be sold at all online retailers. Similarities:

As mentioned, dutasteride and finasteride both fall within the same class of medications and both work in similar ways leading to many similarities. Both showing an effectivity for reducing hair loss and promoting hair regrowth, both reducing the levels of DHT in the body (the hormone linked to hair loss), and both well-tolerated by the many patients who have previously taken either of these medications.

This also means that due to the way that they both work, they show similar side effects:

  • Erectile Dysfunction (ED)
  • Decreased libido
  • Issues with ejaculation
  • Breast tenderness or enlargement (Gynecomastia)
  • Testicular pain or discomfort

All of these side effects are usually mild and often reversible once you have stopped taking either of these medications.

Less common side effects:

Though rare, less common side effects of either medication may occur. These can include, psychological effects such as a risk of depression, skin reactions such as peeling, and breast changes such as lumps, pain, or nipple discharge. If taking either dutasteride or finasteride and experiencing any of these side effects you must report them to your GP or healthcare provider. If you experience a serious allergic reaction to either of these medications, you must call for immediate medical care.

 

Who can take finasteride and dutasteride, and how should it be taken?

  • Men: Both finasteride and dutasteride are prescribed for males only.
  • Age: These medications are suitable for adults only and should not be taken by children or adolescents.

How long does it take to see results from dutasteride or finasteride?

The time it takes to see noticeable results from dutasteride or finasteride for hair loss can vary; both from medication to medication but also from one person to the next.

Generally speaking, patients will start to notice results within a few months of treatment as the rate of shedding reduces. After approximately 6 to 12 months of consistent use, patients will begin to notice new hair growth that is thicker and healthier. After a year of treatment, people usually notice that their hair is fully restored.

Some people may benefit more from a dual approach. This involves using oral or topical minoxidil, an alternative hair loss treatment, together with either finasteride or dutasteride. Please note that dutasteride and finasteride should never be taken together.
